Overview The Program Coordinator/Navigator for Rapid Rehousing and Housing Stabilization Services is responsible for providing case management services and assistance to families and individuals experiencing homelessness. Also responsible for the oversight of the Rapid Rehousing program, including financial monitoring, billing, and reporting. This is a full-time regular position with a workweek expectation of 40 hours per week. Responsibilities This job description is not all inclusive. It is intended to identify the essential duties of the position. Other duties, responsibilities and tasks may be assigned by corps officer/administrator.

Adhere to the guiding principles of The Salvation Army's social services ministries as outlined in Faith In Action with an emphasis on intentional outreach in the area of pastoral care to those we serve. Engage all social service clients (including and not limited to, families/children, etc.) in a way that brings them to the Corps for programming, activities, events, and Pathway of Hope. Conduct Pathway of Hope case management. Responsible for the provision of a full range of case management interventions including engagement of new clients, completion of comprehensive assessments and action plans, referrals to services designed to address identified goals and development of plans to address transition needs and ongoing documentation of progress. Rapid Rehousing Program: Receive referrals and interview families experiencing homelessness. Assist families in finding permanent housing utilizing the HRA, and other identified resources. Assist households in creating a workable service plan with short and long-term goals and objectives. Assist families in all aspects of direct client services, including financial assistance, moving assistance, home visits, crisis intervention and referrals to community resources. Develop collaboration between The Salvation Army and the partner agency to better serve homeless families. Oversight of Rapid Re-housing program budget including monthly spend downs and billings for The Salvation Army and the program partner agency. Provide timely reports to funders, partners and the Director of Residential Services. Pay all invoices in a timely manner. Attend community meetings with program partners, meetings appropriate to the program, and training that will enhance the effectiveness of the program. Housing Navigation and Housing Stabilization Services: Receive referrals and complete client eligibility process to enroll clients in program. Develop a housing focused, person-centered plan based on assessment outcomes. Support the client in identifying strengths, needs, and wants in housing including cultural requirements and/or preferences. Support the client to make an informed choice in their housing transition/sustaining provider. Offer resources for services that support non-housing related goals. Coordinate with all other current service provides to ensure well-rounded, non-duplicative services. Assist the client with understanding their rights to privacy and appeal processes. Annual reassess the person-centered plan as it relates to housing. Additional Responsibilities Advocate on behalf of clients when necessary. Keep accurate and complete client files. Prepare and maintain accurate case notes and statistical information. Report all data on clients into HMIS. All other duties as assigned. Qualifications Bachelors degree in human services related field. If degree is in Social Work, must be licensed by the MN Board of Social Work. Case Management experience working with individuals experiencing homelessness and chemical dependency issues. Experience working with people with disabilities, disability-related issues, and navigating relevant community resources. Knowledge of and experience using Person-Centered Thinking, Person Centered Planning and self-determination, preferred. Familiarity with resources and the rental housing market in Duluth and the surrounding area. Ability to work well with clients of diverse cultural and socioeconomic groups, respect confidentiality, and ability to work within the ethical and legal parameters of social services. Excellent interpersonal and written communication skills required. Ability to accept supervision and work with a team of co-workers. Ability to organize work, set priorities, meet deadlines, and complete tasks with a minimum of direction. Computer literate, with the ability to use Salvation Army approved word processing, spreadsheet, database management, and graphics/desktop publishing software packages. Valid Driver's License and reliable transportation. (Must meet the guidelines provided by the State of residence and the automobile insurance provider as pertaining to corps passenger vans/buses.) Must pass The Salvation Army and MN DHS background checks and meet the MVR drivers qualifications. Must meet MN DHS Housing Stabilization Services provider requirements. Must complete The Salvation Army Caseworker Certification Program within twelve (12) months of hire. Willingness to support the philosophy, goals, objectives, and mission of The Salvation Army. It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment and also understood that the required Safe From Harm training be satisfactorily completed within the first thirty (30) days of employment.
