Job Description:

Job Title : Primary Care Physician (Board Certified/Board Eligible)
Locations : WI or MN
Duration : 1+ Year to long term
Shift: 8:00 am - 4:30 pm

Responsibilities

Provide outpatient primary care and chronic disease management
Perform history & physicals, admissions, evaluations, and treatment plans
Order and interpret diagnostic tests
Coordinate care with specialists and multidisciplinary teams
Participate in acute care stabilization and code response when needed
Complete documentation in VA CPRS/EMR

Required Qualifications

MD or DO
Board Certified/Eligible in Internal Medicine or Family Medicine
Active unrestricted Wisconsin or Minnesota medical license
Current ACLS & BLS certifications
DEA and NPI required
Completion of accredited residency program
