Job Description: Utilizes advanced health assessment, knowledge,
and decision making skills, works independently and in
collaboration with physicians and other health care professionals.
Provides health care to individuals and families, emphasizing
health promotion and disease prevention. They may care for patients
ranging in age from newborn to elderly as noted in age-related
category and area of expertise. Education Qualifications: NP :
Master's degree in nursing PA : Bachelor’s degree in a
health/science-related field Licensure/Certification
Qualifications: The Nurse Practitioner or Physician Assistant
utilizes advanced health assessment, knowledge, and decision-making
skills, while working both independently and in collaboration with
physicians and other health care professionals. The NP/PA will
provide health care to individuals and families, emphasizing health
promotion and disease prevention, caring for patients ranging in
age. Position requires high level of customer service skills to
establish and enhance positive relationships with patients,
co-workers, and others. Work in a beautiful clinic with a
collaborative team working to develop a high quality, efficient
service model. This model promotes a culture of patient
centeredness and staff resilience. The ideal candidate will have
Family Practice experience. POSITION HIGHLIGHTS: Our team consists
of 12 Physicians, 6 Advanced Practice Providers, and additional
staff members, including PharmD, RNs, Clinical Assistants,
Behavioral Health, Addiction Medicine and a Community Health
Worker, with onsite retail Pharmacy, Lab and Radiology. Typical
Schedule: 0.8 FTE Monday – Friday 8:00 a.m. – 5:00 p.m. Estimated
Patients Per Day: 12-15 Ideal candidate will have Family Practice
and Internal Medicine experience. LICENSURE/CERTIFICATIONS:
Master’s or Doctorate degree in Nursing or Master’s degree in
Physician Assistant studies from an accredited program. Current
licensure as a Nurse Practitioner or Physician Assistant in
Minnesota Current certification in BLS National certification, in
applicable area *Hired candidates may be eligible to receive
