Education Qualifications: No Educational Requirements Key Responsibilities: - Caring for vulnerable patients in their home or other community settings to provide health promotion, disease management, care transitions and other services as needed - Work alongside emergency medical services, emergency department and primary care physicians in both the clinic and hospital setting to identify patients who could benefits from community paramedic services - Work under the license of the medical director utilizing established written protocols in conjunction with the patient's primary care physician and the patients' established care plan Schedule is typically Monday - Friday 8am - 4pm. End time can vary between 4-6p, depending on referrals. Flex schedule.
