Job Description: Responsible to assess, plan, implement,
evaluate, supervise and coordinate the home care and hospice care
(if applicable at location) of patients in accordance with
professional standards of nursing and policies and procedures of
Essentia Health. Provides direct patient care to patients requiring
care for medical, diagnostic and emergency conditions. Education

join our team! Sign on Bonus with Commitment $15K for BSN $10K for
ADN (some restrictions apply) Preferred Qualifications for Hospice
Referral RN: Experience in nursing, with emphasis on community
health education/experience Experience supervising caregivers
and/or staff Knowledge of Medicare Home Health & Hospice
regulations Experience with computer databases, work processing,
and spreadsheet programs Strong communication and interpersonal
skills Capable of making independent decisions with the use of good
critical thinking and problem-solving skills Ability to work
autonomously and as a team Experience in health information
management, business, or related field Licensure/Certification
Qualifications: Current nursing licensure in state (s) of
employment. May be required to hold multiple state licensures,
depending upon work location. Basic Cardiac Life Support (BCLS)
certification or ability to become certified within 1 month from
date of hire. Possession of a valid motor vehicle operator's
license as verified through appropriate licensing agency. Proof of
current motor vehicle insurance. 2020-14010 Hospice Program:
Current RN license in the state of Minnesota upon hire. Ability to
obtain RN license in Wisconsin within 3 months of hire. Employee
