Job Description:
Job Description: The Care Management Associate supports the RN
Care Coordinator utilizing excellent organizational and
communication skills and showing a personal commitment to providing
compassionate care. Education Qualifications: Key Responsibilities:
Provides communication with patients, care teams, and other
stakeholders to ensure that all relevant information is available
to provide the best possible quality of care Assists the RN Care
Coordinator with scheduling appointments, coordinating care,
providing follow-up services to patients, submits data to health
plans, and any other duties as assigned Tracks and maintains
patient records and insurance information Provides administrative
support including data entry, documentation in the Electronic
Medical Record, and other duties as required Collaborates with
other departments to ensure the efficient delivery of care while
demonstrating a strong commitment to providing compassionate and
quality care Required Qualifications: Medical Secretary, Health
Unit Coordinator, or similar program Minimum of 2 years’ experience
in a medical setting if no education meets the Education
