Job Description:

Physical Therapist (PT) needed to provide home health care services to patients recovering from illness or injury.
Aveanna Healthcare is a leading provider of home health care services, and we are seeking a skilled Physical Therapist to join our team.
Key Responsibilities:

• 
  
• Conduct initial and ongoing assessments to evaluate patient mobility, coordination, strength, and current level of function.
  
• Develop and implement physical therapy plans to achieve patient goals and outcomes.
  
• Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes.
  
• Deliver physical therapy services in accordance with prescribed physician orders.
  
• Maintain accurate and timely documentation of patient progress and modify treatment plans as needed.
  
• Collaborate with healthcare team members to ensure effective patient care and discharge planning.
  
• Supervise and educate other personnel as applicable.

    Requirements:
    
    • 
      
    • Degree in Physical Therapy from an accredited university/program.
      
    • Current, unrestricted state license as a Physical Therapist in the state of practice.
      
    • Valid CPR certification.
      
      Preferred Qualifications:

      • Proven relationship-builder.
        
      • Experience in Home Health Therapy and completion of OASIS documentation.
