We are looking for a full-time (1.0 FTE) Registered Nurse Care Manager to join our integrated team at our Iron River Clinic location. The RN Care Manager will manage high-risk and chronically ill patients, in collaboration and continuous partnership with patients and their family/caregiver(s), clinic/hospital/providers and specialty providers and staff, and community resources in a team approach. The schedule for this position is Monday-Friday, no weekends, no holidays, and no overnights.Registered Nurse duties include but are not limited to:

  • Conduct in-office patient visits as outlined in care plan and based on patient's health status and self management interests
  • Promote timely access to appropriate care to reduce emergency room utilization and hospital readmissions
  • Increase utilization of preventative care
  • Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider and/or specialty care provider and family/caregiver(s)
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals
  • Increase patients' ability for self-management and shared decision-making
  • Provide medication reconciliation
  • Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care cost
  • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources
  • Facilitate patient access to appropriate medical and specialty providers
  • Educate patient and family/caregiver(s) about relevant community resources
  • Facilitate and attend meetings between patient, family/caregiver(s), care team, and community resources, as needed
  • Assist with the identification of high-risk patients (the chronically ill and those with special health care needs), and add to care coordination tracking
  • Attend all Care Coordinator training courses/webinars and meetings - Provide feedback for the improvement of the Care Coordination Program
  • Follow protocols, clinical policy and procedure in collaboration with the Primary Care Provider/Specialty Care provider for chronic care management.
  • Participates in quality improvement efforts by; utilizing dashboards, tracking tools and providing quality reporting data monthly
  • Represents the clinical team and is a point of contact for team in meetings and planning activities; also point of contact for staff.
  • Performs nursing assessment and triage.
  • Follows clinical guidelines protocols and policies related to direct patient care.
  • Complete refill requests for controlled substances under direction and approval from prescribing provider.

    Requirements:
    • RN with two to three years related experience and/or training; Evidence of essential leadership, communication, education, and counseling skills.
    • Current, valid, and unrestricted Wisconsin license to practice as a Registered Nurse.
    • Ability to pass a Wisconsin Healthcare background check.
    • Wearing a mask is a requirement for all of our positions.Preferred:
      • Local knowledge about connections to community health care and social welfare resources is desirable.
      • Experience with Electronic Health Records.Wage:
        • $24.26-$28.75/hour depending on experienceDeadline to Apply:
