CASE MANAGER - FHPAP/HOUSING NAVIGATOR

Job Description:

Position Summary The Salvation Army Northern Division Duluth Corps is doing the most good by providing holistic, person-centered services to the local community. Services include worship and spiritual care; short and long-term case management; housing; food, clothing, utility, and rental assistance; youth programming; and connections to community partners. The Case Manager of the Family Homeless Prevention and Assistance Program (FHPAP)/Housing Navigator provides targeted case management services to empower participants in their journey to secure and maintain housing. The Case Manager provides participants with supportive services and financial assistance to minimize the number of homeless days. 35 hours/week, $21.00/hour 215 South 27th Avenue West, Duluth, MN 55806 Essential Functions (approximate time spent)

  • Conduct strength-based, person-centered, holistic case management for FHPAP participants, including processing financial assistance and providing housing navigation. (60%)
  • Maintain up-to-date, accurate participant information and financial transactions in casefiles and through database entry and reporting. (15%)
  • Engage in internal and external relationship building to develop new resources, best serve participants, and achieve program goals. (10%)
  • Maintain a catalog of local community housing resources and providers. (5%)
  • Promote and further Salvation Army and FHPAP goals and objectives through internal and external activities. (5%)
  • Assist with seasonal and special events. (5%)
    Education and Experience
    • Bachelor's degree in a relevant field. Instead of a degree, education, and experience may be considered.
    • Must be licensed by the state board if the degree is in social work.
    • Previous social services experience required.
    • Experience conducting long-term case management preferred.
    • Must pass background checks, possess a valid driver's license, and meet MVR requirements.

        • Must be able to communicate clearly.
        • Standard business hours with occasional evenings and weekends.
        • Occasionally lift 20 pounds
          Supervisory Responsibilities None. Driving/Travel Requirements Regularly drive agency vehicles locally to support the mission.
